We now offer a care management program for individuals with Medicare, called ChartSpan.

Care management is a Medicare program that gives you access to remote care in between your regular office visits. The program’s goal is to keep you healthy, offer you assistance, and prevent any conditions you have from worsening.

Care management benefits include:

• • • • A 24/7 care line where you can reach clinicians and nurses
      • A customized care plan and care goals
      • Assistance with setting appointments and filling prescriptions
      • Connections to community resources, like transportation assistance

A care management program reduces unnecessary hospital visits, ER visits, and overall healthcare costs. If you’re eligible, a program representative will call to ask if you would like to join the program. You can also ask your provider for more information about care management and whether you may be eligible.
