Transitional Care Management
River Valley Family Health Center wants to be there for you any time you leave a hospital or other overnight medical facility. We work with the hospital you were discharged from to ensure you have a follow-up appointment with your primary care provider here at River Valley. Often, you will be set up with an appointment before you even leave the hospital. River Valley medical staff will gather information about any new medications you may have been prescribed at the hospital, ensure you have transportation for follow up care and provide community resources related to any new diagnosis you may have received.

River Valley Family Health Center is now offering appointments specifically for individuals and families who need guidance surrounding

• Diabetic education; 2 Certified Diabetic Educators (CDEs)
• Annual wellness visits
• Preventative health screenings: colorectal cancer, breast cancer, skin cancer, cervical cancer, A1-C, blood glucose, and blood pressure.
• Sports physicals ($25)
• Well child checks for newborns and all children
• Women’s health
• Skin cancer screenings
• Adult and child vaccines, all ages and ability to pay
• Lesion removal
• Joint Injections
• Dry Needling
• Diabetic retinopathy machines
• In house labs working with Labcorp
• Vaccines
• Medication Assisted Treatment: River Valley provides Medication Assisted Treatment (MAT) for opioid and alcohol use disorder. The MAT program involves the use of prescription medications in combination with comprehensive counseling and rehabilitation support services including peer recovery services and care coordination (connecting patients to local resources such as immediate and long-term housing, food, vocational training and transportation).
The ultimate goal of our MAT program is full recovery, including the ability to live a self-directed life.

Medicare Annual Wellness Appointments
River Valley Family Health offers Medicare Annual Wellness appointments for people who are 65 and older who also have Medicare. At these visits, our Registered Nurses can identify preventative screenings that Medicare will cover. We will help you get these screenings completed and results will be delivered back to your primary care provider for review at future visits. Medicare patients will get to meet for extended appointment times with Registered Nurses to talk about health goals, specific tests and labs, and review past medical and surgical history. These visits are not a physical exam, but more of a conversation about preventative care and overall wellness.
