You must have an appointment. This is to ensure that we have the vaccine needed in stock, as well as all the required information.
Do we have to be a patient at River Valley to receive vaccines?
No, you do not, but we would love to have you as our patient.
What do I need to bring to my appointment?
- For a child- you must bring a copy of their most recent Well Child Check with the vaccines needed listed in the providers note. Date of visit must be within the last 12 months.
- For an adult– you must bring a copy of your most recent Adult Annual Wellness Visit/ Annual Physical with the vaccines needed listed in the providers note. The date of the visit must be within the last 12 months.
