This is a new policy and needs to be filled out by the parents of any minor (17 and under) that would like to have someone else bring their child to an appointment with River Valley Family Health Centers.
Please download and fill out digitially, print, sign and bring into a River Valley office.

Olathe Primary Care Clinic Wednesday's 8:15am – 6:00pm
Delta Convenient Care Clinic: Mon, Wed, Thurs, Fri 8:00am – 6:00pm, closed for lunch 1:00pm – 1:30pm
Montrose Convenient Care Clinic: Mon, Tues, Thurs, Fri: 8:00am – 6:00pm; closed for lunch 1:00pm – 1:30pm
Montrose Pharmacy: 8:30am – 6:30pm M-F and 9:00am – 1:00pm on Saturday
Delta Pharmacy: 8:30am – 6:30pm M-F and 10:00am – 2:00pm Sunday
Pharmacies are closed for lunch from 1:00pm – 1:30pm in both locations.
